Preschool & Grade School
Please follow the following steps:
1. Fill out the Student Application Form.

2. Click the “save” button to submit your application.
3. After submission, the applicant will receive a confirmation email. (For the Next Procedure).
Admission Requirements:
- PSA Live Birth Certificate (original and photocopy).
- Baptismal Certificate (original and photocopy).
- Parent’s Marriage Certificate (original and photocopy).
- Medical Certificate.
- Report Card (Form 138) except Toddlers/ Nursery/ Junior Kinder (Original Copy).
- Permanent Record (Form 137) except Toddlers/ Nursery/ Junior Kinder (Original copy).
- Certificate of Good Moral Character except Toddlers/Nursery/ Junior Kinder.
- 4pcs 1×1 picture with white background taken within the last six months.
- Other documents as may be required.

- Proof of billing (Residents of Hiyas Homes, Fortune Village 5, 6, and 7 subdivisions)
Grade School
- Honors and Ranking Certification signed by the principal
- Proof of billing (Residents of Hiyas Homes, Fortune Village 5, 6, and 7 subdivisions)
