2. Click the “save” button to submit you application.
3. After submission, the applicant will receive a confirmation email. (For the Next Procedure).
Admission Requirements:
- PSA Birth Certificate (Original & Photocopy)
- Parents’ Marriage Certificate (Original and Photocopy)
- Report Card – Form 138 (Original Copy)
- Certificate of Good Moral Character
- Medical Certificate
- Permanent Record (Form 137) / SF10 (Original copy)
Other
- Honor’s Certification signed by the Principal
- ESC certification (If ESC granting) (Original copy)
- Proof of billing (Residents of Hiyas Homes, Fortune Village 5, 6, and 7 subdivisions)
- Recommendation Form ( Guidance Counselor)
Non-Resident Alien (NRA)
- Alien Certificate of Registration/ Special Immigration
- Resident Visa/ Immigration Certificate Registration
- Authenticated Scholastic Records from school of origin
- Original and Photocopy of Passport
- Other Documents may be required.
