- Latest Assessment from Developmental Pediatrician
- Speech, Occupational and Behavioral Therapy Reports
- Other Documents may be required

- Alien Certificate of Registration/ Special Immigration
- Resident Visa/ Immigration Certificate Registration
- Authenticated Scholastic Records from school of origin
- Original and Photocopy of Passport
- Other Documents may be required.
